Job summary

The Hospitals of Providence - Sierra Campus is seeking an Admissions Liaison to perform comprehensive pre-admission assessments for inpatient rehabilitation referrals. This role coordinates patient information and streams it to the Medical Director for timely review, while maintaining clear communication with referral sources throughout the process.

Qualified candidates will hold a bachelor's degree or higher licensure, at least 2 years of direct rehabilitation experience, and be licensed as PT,

Qualifications

• Bachelor's degree or higher licensure as required by the state.
• 2 years of clinical experience, including direct patient care in a rehabilitation setting.
• Licensed PT, RN, OT or SLP; AHA BLS certification.

Responsibilities

• Conduct pre-admission screenings for rehab referrals to determine appropriateness for inpatient rehab.
• Develop and implement patient attraction strategies to promote rehab program features and benefits.
• Coordinate the referral process and communicate with referral sources at all stages.
