Job summary

Lifepoint Health, through the Rehabilitation Institute of South San Antonio, seeks a Clinical Liaison (RN/LVN/PT/OT/SLP/RRT/LMSW) to drive admissions and coordinate pre-admission assessments. This PRN role requires travel to referring facilities and close collaboration with physicians, case managers, and payors.

Education: Bachelor's degree preferred; active licensure in a clinical field is required. Strong communication and EMR skills are essential for success in this role.

Qualifications

• Bachelor's degree preferred.
• Current clinical licensure required (e.g., PT, OT, SLP, RT, RN, LVN, LMSW) per state requirements.
• Prior marketing/sales experience preferred; prior clinical experience with demonstrated assessment skills and EMR proficiency preferred; formal sales training preferred.

Responsibilities

• Builds and maintains referral relationships to drive admissions.
• Coordinate intake and pre-admission screenings.
• Educate patients and families on services and participation.
• Serve as external contact for case managers, discharge planners, payors, and hospital partners.
• Conduct on-site clinical assessments (typically at referring facilities) to determine appropriateness for admission.
• Document screenings, decisions, and communications in EMR and tracking systems.
• Perform other duties as assigned.
