Job summary

Adelante Healthcare is seeking a Registered Nurse to guide patients and families through the care system, providing education, advocacy, and goal-setting for prevention and self-management. The role may include triage for urgent care inquiries to determine whether outpatient services are appropriate or require escalation.

Ideal candidates have active AZ nursing licensure, 2–5 years of clinical experience, strong record-keeping skills, and solid computer literacy including MS Office and a patient

Qualifications

• Graduation from an accredited nursing program.
• AZ State Board of Nursing licensure in good standing.
• Experience with clinical record-keeping systems.
• Experience supervising others in a clinical setting.
• Advanced computer proficiency with Microsoft Office and patient management systems.
• Immunization requirements met.
• Valid Arizona driver’s license.
• 2–5 years of clinical experience.
• CPR and AED certification per AHA/Red Cross guidelines.
• Valid Level One Fingerprint Clearance Card.

POSITION SUMMARY

This Registered Nurse functions to assist all patients and their families through the healthcare system by acting as a health educator and patient advocate. They will also work with patients to develop preventive and self-management goals to improve their health. Additionally, this nurse position may provide triage services to patients calling for urgent care needs to provide medical advice and identify if outpatient services are appropriate or if they should be escalated to a higher level of care. This position is essential to Adelante’s team-based primary care medical home model of care that supports coordinated, patient-centered, comprehensive health care to all.

ESSENTIAL SKILLS AND EXPERIENCE:

Graduation from an accredited nursing program

Licensed with AZ State Board of nursing; license in good standing

Demonstrated understanding of clinical methods and techniques used in record-keeping systems

Demonstrated ability in implementing the principles and practices of supervision

Advanced computer efficiency in Microsoft Office suite (Word, Excel, PowerPoint, Outlook) and experience in Patient Management System

Meet the organization immunization requirement

Valid Arizona driver’s licenseTwo (2) to five (5) years of experience in a clinical setting

Certification to perform cardiopulmonary Resuscitation for the Health Care Professional (CPR) and AED through courses that follow the guidelines from the American Heart Association and Red Cross (cognitive and skills evaluations)

Valid Level One Fingerprint Clearance Card issued by the Arizona Department of Public Safety for all specialty behavioral health locations
