The Registered Nurse Supervisor is responsible for the overall leadership of clinical nursing services, and assumes leadership of nursing personnel (RN, LPN, at designated site(s). They will clinically supervise Registered Nurses, License-Practical Nurses, as assigned, and act as liaison with accredited Nursing programs. In collaboration with the Senior Manager of Nurse and Quality Improvement & Chief Clinical Operations Officer, the Nurse Supervisor will play a vital role in the preparation for on-site clinical and operational audits, as well as the development and monitoring of quality improvement projects and Uniform Data System (UDS) and Health Effectiveness Data Information Set (HEDIS) quality metrics. The Registered Nurse Supervisor is directly responsible for the infection prevention and control program and all activities.

Performs triage calls using approved clinical decision support tools for the adult primary care, OB, and Peds populations. Participates in team meetings, designated “Specialty Team” meetings, and quality and process improvement initiatives. Helps facilitate health and disease patient education (including adults, peds, and OB populations) sessions. Coordinates continuity of patient care with external healthcare organizations and facilities such as referrals to outside specialties and ongoing two-way communication between providers and care teams. Develop relationships with external healthcare organizations and facilities in efforts to find additional resources. Coordinate outreach efforts to high-risk patients and hospital discharge and admissions. Collaborate with health plans, hospitals, and skilled nursing facilities for transitional needs. Conduct comprehensive, preventative screening for patients and/or assists all support staff in daily patient interactions as needed. Completion of Nurse Visits including but not limited to POCT items (flu, strep, UA, HCG testing). Helps evaluate clinical care, utilization of resources, and development of new clinical tools, forms, and procedures. Manages insulin titration for the diabetic population, which can include outbound calls to follow up. Excellent customer service and communication skills Performs assessment and delivers patient-specific education plan.
