CareDiem Home Health is seeking compassionate LPNs to provide skilled home health nursing services to patients in Bureau County. Deliver nursing care under a physician's order and travel to patient homes to support safe, in-home care.

Applicants should hold an active Illinois LPN license, have 3+ years of nursing experience (home health preferred), and CPR/BLS certification. Reliable transportation and a strong clinical mindset are essential for success.

What We’re Looking For:

• Current LPN license in the state of Illinois
• 3+ year nursing experience (home health preferred)
