Job summary

CareDiem Home Health in Illinois is seeking compassionate LPNs to provide skilled nursing services to patients in the LaSalle County area. You will travel to patient homes, perform assessments, administer medications, and educate patients and families.

What We’re Looking For:

• Current LPN license in the state of Illinois

• 3+ year nursing experience (home health preferred)

• CPR certification (BLS)

• Reliable transportation, valid driver’s license, and auto insurance

• Strong clinical judgment, communication, and independence
