In this critical role, you will serve as the centralized point of contact for all acute EMS patient transfers into, and across, our organization – ensuring safe, timely, and seamless transitions of care for patients and providers across the region.
As a Transfer Center Coordinator, you play a vital role in patient access and hospital flow. EMS interfacility transport is a complex, high‑risk component of healthcare delivery that is critical for patient access to appropriate care. Effective transfers depend on accurate triage, proper transport modality selection, and pre‑established coordination between facilities. You will collaborate with hospitals, physician offices, long‑term care facilities, emergency departments, and internal clinical teams to ensure patients are transferred at the right time, to the right place, and at the appropriate level of care.
Part-Time Two 12 - hour shifts per week Night Shift: 7:00 PM. - 7:30 AM Every third weekend rotation
