Job summary

Surry Health Center by Harborview is seeking a dedicated Registered Nurse to support CNAs and coordinate with the IDT to ensure high-quality resident care. The role emphasizes timely assessments, documentation, and physician collaboration in a faith-based skilled nursing setting.

Qualifications

• Active RN license in good standing in the state of practice.
• Experience in a skilled nursing or long-term care setting is preferred but not required—we’ll invest in the right heart and mindset.

Responsibilities

• Supervise and support CNAs under the direction of the Director of Nursing.
• Collaborate with the interdisciplinary team to assess, plan, implement, and evaluate individualized care plans.
• Ensure baseline and periodic assessments are completed on time.
• Prepare for and facilitate physician rounds by flagging concerns and preparing documentation.
• Initiate consultations or referrals as appropriate based on resident needs.
• Perform administrative tasks including medical documentation, evaluations, and reports.
• Deliver nursing care within scope of practice and aligned with policy and clinical standards.
• Monitor residents for changes in condition and notify providers as needed.
• Perform other duties as assigned by the Director of Nursing.
