Job summary

Qualifications

Responsibilities

• Administer Medication, including intramuscular injections, as ordered by MD.
• Conduct substance abuse assessments to determine appropriateness for treatment.
• Follow up on health-related referrals and track patient health issues.
• Provide patient education and counseling on health-related issues.
• Provide staff education around health-related issues.
• Complete documentation and reports in a timely manner.
• Participate in staff meetings, case conferences and unit meetings.
• Ensure compliance with Medicaid plan and agency policies.
• Coordinate care with clinical staff and act as health care liaison.

Salary Range: $90,000.00 To $120,000.00 Annually

The RN Supervisor assesses health related needs of the clients by monitoring ongoing health and wellbeing. The RN Supervisor directly provides and/or coordinates health services and health-related activities for clients. The RN Supervisor provides on-call nursing for times when we do not have a nurse on site and serves as the health care coordinator. The RN Supervisor provides clinical supervision and direction to the nursing team.

• Administer Medication, including intermuscular injection, as ordered by MD.
• Conduct substance abuse assessments to determine appropriateness for participation in treatment, need for physical exam, or other medical services.
• Follow up on any health related referrals made.
• Maintain a tracking system to monitor patient health issues, medical appointments, and prescribed medications (Internal/External).
• Provide patient education and counseling around health related issues.
• Provide staff education around health related issues.
• Complete all needed documentation and reports in a timely manner. Informs program leadership if any problems or anticipated inability to meet deadlines.
• Actively participate in staff meetings, case conferences, and other unit meetings.
• Prioritize and plan work time efficiently; effectively managing competing demands and dealing with changes, delays, and unexpected events.
• Maintain communication and teamwork with clinical staff to integrate and enhance clinical services.
• Conduct toxicology testing as needed.
• Exercise due diligence in the delivery of quality care in line with agency Medicaid Compliance Plan and systems of accountability.
• Ensure communication through the compliance officer and/or the supervisory structure of any violation of non-compliance with the agency’s Medicaid compliance plan, and personnel policy and procedure.
• Identify the current health needs of the patient population and implement activities to address service and informational deficits.
• Perform other duties assigned as relates to role.
• Must be a NYS Registered Nurse (RN).
• Experience working in a substance abuse treatment setting or special populations required.
• Supervisory experience a plus.
• Experience with AVATAR electronic health records a plus.
