Job summary

Creekside Post Acute in Hopewell, VA is seeking a compassionate Registered Nurse (RN) with wound care expertise to join our skilled rehab team. You will focus on wound prevention, wound care, ostomy care, and incontinence management to support residents' health and comfort.

As part of an interdisciplinary team, you will develop and execute individualized care plans, document care activities, and educate residents and families on post-operative and ongoing wound care.

Responsibilities

• Develop and implement policies and procedures to manage residents with draining wounds, fistulae, and/or tubes
• Provide education to residents undergoing surgery resulting in an ostomy, including pre- and post-operative assessments and resident and family education
• Make written or oral reports concerning the activities of your shift, as required
• Ensure all nursing service personnel comply with procedures set forth in the Nursing Service Procedures Manual
