Job summary

Pillar of Cedar Valley Nursing and Rehabilitation is seeking a Wound Care Registered Nurse to provide specialized nursing care for patients with acute and chronic wounds. You will assess, evaluate, treat, and monitor wounds with physicians, nurses, patients, and caregivers.

The ideal candidate demonstrates strong clinical judgment, attention to detail, and knowledge of evidence-based wound prevention and treatment practices.

Qualifications

  • Current RN license in the applicable state.
  • Wound care experience preferred.
  • Ability to work with interdisciplinary teams.

Responsibilities

  • Perform comprehensive wound assessments and document findings.
  • Develop and implement individualized wound care plans.
  • Provide wound treatments including dressings and debridement as permitted.
  • Monitor healing and adjust plans as clinically needed.
  • Identify risk factors for pressure injuries and infection.
  • Collaborate with physicians, dietitians, and therapists.
  • Educate patients, families, and caregivers on wound care and prevention.
  • Guide nursing staff on wound assessment and treatment techniques.
  • Maintain accurate clinical documentation and progress notes.
  • Follow infection control, safety, and facility policies.
  • Participate in quality improvement for wound outcomes.

Position Summary

The Wound Care Registered Nurse (Wound RN) is responsible for providing specialized nursing care to patients with acute and chronic wounds. The Wound RN assesses, evaluates, treats, and monitors wounds while developing individualized plans of care in collaboration with physicians, nursing staff, patients, and caregivers.

The ideal candidate demonstrates strong clinical judgment, attention to detail, excellent communication skills, and knowledge of evidence-based wound prevention and treatment practices.

Job Responsibilities
  • Perform comprehensive wound assessments, including evaluation of wound type, size, depth, drainage, tissue characteristics, and surrounding skin.
  • Develop and implement individualized wound care plans based on patient needs and current clinical standards.
  • Provide wound treatments, including dressing changes, debridement as permitted by scope of practice, and other prescribed therapies.
  • Monitor wound healing and modify treatment plans as clinically appropriate.
  • Identify and address risk factors for pressure injuries, skin breakdown, infection, and delayed wound healing.
  • Collaborate with physicians, advanced practice providers, dietitians, physical therapists, and other members of the interdisciplinary care team.
  • Educate patients, families, and caregivers regarding wound care, pressure injury prevention, nutrition, mobility, skin care, and signs of infection.
  • Provide guidance and education to nursing staff regarding wound assessment, prevention, and treatment techniques.
  • Maintain accurate and timely clinical documentation, including wound measurements, photographs when applicable, treatment plans, and progress notes.
  • Follow infection prevention, safety, regulatory, and facility policies.
  • Participate in quality improvement initiatives related to wound outcomes and patient safety.
  • Maintain current knowledge of evidence-based wound care practices, products, and technologies.

Qualifications
  • Current Registered Nurse (RN) license in the applicable state.
  • Associate or Bachelor's degree in Nursing preferred.
  • Previous clinical nursing experience required; wound care experience preferred.
  • Strong assessment, critical-thinking, organizational, and communication skills.
  • Ability to work collaboratively with patients, families, physicians, and interdisciplinary teams.
  • Ability to prioritize care and manage multiple patients and clinical responsibilities.
