Job summary

Creekside Post Acute in Hopewell, VA is seeking a compassionate Registered Nurse (RN) Wound Care Nurse to join our team. You will focus on wound prevention and care, ostomy support, and incontinence management for residents.

You will work with an interdisciplinary team to develop effective care plans, educate residents and families, and ensure high-quality clinical practice in a supportive environment.

Qualifications

• Current RN license in Virginia
• Experience with wound care and ostomy care preferred
• Strong assessment and documentation skills

Responsibilities

Job Summary:

Creekside Post Acute, a leading provider of short-term and long-term skilled rehab services, is seeking a compassionate and skilled Registered Nurse (RN) Wound Care Nurse to join our team in Hopewell, VA. As a Wound Care Nurse, you will play a vital role in providing high-quality care to our residents, focusing on wound prevention, wound care, ostomy care, and incontinence management.

Responsibilities:

• Develop and implement policies and procedures to manage residents with draining wounds, fistulae, and/or tubes
• Provide education to residents undergoing surgery resulting in an ostomy, including pre- and post-operative assessments and resident and family education
• Make written or oral reports concerning the activities of your shift, as required
• Ensure all nursing service personnel comply with procedures set forth in the Nursing Service Procedures Manual
