Job summary

Interim HealthCare Inc. is seeking a Home Health Registered Nurse (RN) to deliver skilled nursing in patients' homes across Piedmont, Pelzer, Williamson and surrounding Anderson County.

You will collaborate with physicians and the care team to support safe recovery and long-term health. The role requires SC RN licensure (or Compact) and at least one year of nursing experience; home health and OASIS familiarity are preferred.

Qualifications

• Graduate of an accredited Registered Nurse (RN) program.
• Active South Carolina RN license or Compact License.
• Minimum one year of nursing experience.
• Experience in Home Health, Medical Surgical, Critical Care, or Case Management preferred.
• Familiarity with OASIS documentation preferred.

Responsibilities

• Provide skilled, patient-centered care in the home.
• Perform comprehensive patient assessments, including OASIS documentation.
• Develop, implement, and adjust individualized plans of care.
• Monitor patient condition and communicate changes promptly to physicians.

Shift: Full-time, Friday 4:30pm — Monday 8:30am. Location: Piedmont, Pelzer, Williamson. Anderson County up to West Greenville.

What You’ll Do

• Provide skilled, patient centered care in the home while partnering with physicians and the interdisciplinary team to support safe recovery and long-term health
• Perform comprehensive patient assessments, including accurate OASIS documentation
• Develop, implement, and adjust individualized plans of care
• Monitor patient condition and communicate changes promptly to physicians to prevent complications
• Educate patients and caregivers on medications, treatments, disease management, and home safety
• Coordinate care with therapists, aides, and other providers to ensure seamless, compliant care
• Prepare patients and families for safe, successful discharge and document care accurately in the EMR

What You’ll Need

• Graduate of an accredited Registered Nurse (RN) program
• Active South Carolina RN license or Compact License
• Minimum one year of nursing experience
• Experience in Home Health, Medical Surgical, Critical Care, or Case Management preferred
• Familiarity with OASIS documentation preferred
