Job summary

Home Health Care, Inc. is seeking an experienced Weekend Start of Care RN to perform SOC admissions in patients' homes across the Twin Cities area. You will conduct comprehensive assessments, develop care plans, and document OASIS and related data on a weekend schedule.

The role focuses on admission activities, clinical independence in patients' homes, and coordination with the care team to ensure a smooth start of care. Mileage is reimbursed; weekend commitment is required.

Qualifications

• Active Minnesota RN license.
• Current CPR certification.
• Prior home health experience required.
• Strong OASIS and Start of Care knowledge.
• Excellent clinical assessment and documentation skills.
• Ability to work independently in patients' homes.
• Valid driver’s license with reliable transportation.

The Start of Care nurse has an incredibly important role in home health. You are often the first clinician entering the patient's home. You'll assess your condition, identify immediate concerns, determine what services are needed, educate the patient and family, and establish the clinical foundation for the entire episode of care. For an experienced home health nurse, it's an opportunity to use the full range of your assessment, OASIS, care coordination, and patient education skills.

What You'll Do

What We're Looking For

• Active Minnesota Registered Nurse (RN) license.
• Current CPR certification.
• Previous home health experience required.
• Strong working knowledge of OASIS and Start of Care admissions.
• Excellent clinical assessment and documentation skills.
• Confidence working independently in patients' homes.
• Valid driver's license, active auto insurance, and reliable transportation.
