Job summary

Avera Health is seeking a Registered Nurse for Avera@Home Mitchell to provide comprehensive nursing care to patients in home settings or facilities. You will follow ANA standards and be responsible for the nursing outcomes through assessment, planning, delivery, and evaluation, while coordinating with patients and families.

The role involves days with occasional night shifts, weekends, and on-call. A sign-on bonus up to $10,000 and potential relocation assistance are available.

The pay range for this position is listed below. Actual pay rate dependent upon experience.

$31.00 - $46.25

Position Highlights

M-F: 8-4:30 occasional weekend & call

A Brief Overview

Responsible for utilizing the nursing process to provide nursing care for Avera@Home patients following the ANA Standards of Clinical Nursing Practice. The Nurse assumes responsibility for the outcome of nursing care provided to patients in their homes or in facilities. The Nurse demonstrates the knowledge and skills necessary to provide exceptional care for the population served.

• Collects patient health data.
• Analyzes the assessment data in determining nursing diagnoses/problem focus.
• Identifies expected outcomes/goals individualized to the patient.
• Develops a plan of care that prescribes interventions to attain expected outcomes.
• Implements the interventions identified in the plan of care.
• Evaluates the patient's progress toward attainment of the outcomes
• Systemically evaluates the quality and effectiveness of nursing practice.
• Collaborates with the patient, significant others, and healthcare providers in providing patient care.
• Utilizes research/evidence-based findings in practice if appropriate.
• Considers factors related to safety, effectiveness, cost in planning, and delivering patient care.
