Job summary

Avera Health is seeking a Registered Nurse for Avera at Home in Aberdeen to provide comprehensive nursing care in patients' homes. You will collect data, formulate individualized care plans, implement interventions, and evaluate progress in collaboration with families and the care team.

The role emphasizes clinical nursing practice per ANA standards, safety, cost considerations, and coordination with multidisciplinary teams. A valid RN license and a valid driver’s license are required.

Work where you matter.A Brief OverviewResponsible for utilizing the nursing process to provide nursing care for Avera@Home patients following the ANA Standards of Clinical Nursing Practice. The Nurse assumes responsibility for the outcome of nursing care provided to patients in their homes or in facilities. The Nurse demonstrates the knowledge and skills necessary to provide exceptional care for the population served.What you will doCollects patient health data.Analyzes the assessment data in determining nursing diagnoses/problem focus.Identifies expected outcomes/goals individualized to the patient.Develops a plan of care that prescribes interventions to attain expected outcomes.Implements the interventions identified in the plan of care.Evaluates the patient’s progress toward attainment of the outcomesSystemically evaluates the quality and effectiveness of nursing practice.Collaborates with the patient, significant others, and healthcare providers in providing patient care.Utilizes research/evidence-based findings in practice if appropriate.Considers factors related to safety, effectiveness, cost in planning, and delivering patient care.Essential QualificationsThe individual must be able to work the hours specified.
