Job summary

Avera at Home is seeking a Registered Nurse to provide in-home nursing care using the nursing process and ANA standards. You’ll work with a multidisciplinary team to deliver high‑quality patient care in patients’ homes or facilities.

The role involves data collection, care planning, implementing interventions, and evaluating patient progress while maintaining safety and cost awareness. A sign‑on bonus and possible relocation bonus are available.

Location: Avera at Home Mitchell Worker Type: Regular Work Shift: Primarily days with night, weekend and holiday on-call involvement (United States of America) Pay Range: The pay range for this position is listed below. Actual pay rate dependent upon experience. $31.00 - $46.25

Position Highlights

• M-F: 8-4:30 occasional weekend & call

A Brief Overview

Responsible for utilizing the nursing process to provide nursing care for Avera@Home patients following the ANA Standards of Clinical Nursing Practice. The Nurse assumes responsibility for the outcome of nursing care provided to patients in their homes or in facilities. The Nurse demonstrates the knowledge and skills necessary to provide exceptional care for the population served.

• Collects patient health data.
• Analyzes the assessment data in determining nursing diagnoses/problem focus.
• Identifies expected outcomes/goals individualized to the patient.
• Develops a plan of care that prescribes interventions to attain expected outcomes.
• Implements the interventions identified in the plan of care.
• Evaluates the patient’s progress toward attainment of the outcomes Systemically evaluates the quality and effectiveness of nursing practice.
• Collaborates with the patient, significant others, and healthcare providers in providing patient care.
• Utilizes research/evidence-based findings in practice if appropriate.
• Considers factors related to safety, effectiveness, cost in planning, and delivering patient care.
