Job summary

UP Health System - Portage, part of Lifepoint Health, seeks a Clinic RN to join a mission-driven nursing team. You will deliver comprehensive patient care in a hospital setting, document thoroughly, and coordinate with physicians, nursing homes, and families.

The role emphasizes advocacy and education delivered with cultural sensitivity. Requirements include a Michigan RN license, an associate degree (bachelor's preferred), and BLS certification, with vaccination or declination on file.

Qualifications

• Current Michigan RN license or able to obtain within 90 days of hire.
• Associate degree from an accredited nursing program; bachelor's preferred.
• Basic Life Support certification required (available upon hire).
• Annual Influenza vaccination or declination required.

Responsibilities

• Performs plan of care intervention, including medication administration and clinical treatments.
• Documents patient care given with accuracy.
• Monitors and communicates patient condition to care team, physician, and family.
• Collaborates across disciplines to coordinate patient care, transfers and discharges.
• Provides education to patient and family, appropriate to age and culture.
• Advocates for the patient’s physical and emotional well-being.
• Delegates patient care activities to qualified staff.
• Performs prior authorizations for tests, procedures, and medications as needed.

How You'll Contribute

A Clinic RN who excels in this role:

• Performs plan of care intervention, including medication administration, specimen collection, clinical treatments, as well as other medical care treatment.
• Documents patient care given.
• Monitors, records, and communicates patient condition as appropriate to care team, physician, nursing home, patient, and family.
• Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.
• Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances.
• Works as an advocate for the physical and emotional well-being of the patient.
• Delegates patient care activities to appropriately qualified staff.
• Performs prior authorizations on tests, procedures, medications, etc. that are required by the patient's insurance.
• Performs other job duties as assigned.

What We're Looking For

Applicants should have a current Michigan RN license (or attain within 90 days of hire) and possess an associate degree from an accredited nursing school (bachelor's degree preferred). Additional requirements include:
