Job summary

Dartmouth-Hitchcock Health in Lebanon, NH, seeks an experienced Registered Nurse to join the System Staffing Office as a Long-Term Care System Float. This role supports subacute care with an 8/9 patient ratio and includes 3x12 hour shifts with nights or D/N rotation.

Prior Medical-Surgical experience and LTC/subacute float ability are required, and the incumbent will work across Hanover Terrace with acute/subacute patient needs. RN licensure in NH/VT and BLS are required.

Qualifications

• Graduate from an accredited nursing program; RN licensure in NH/VT required.
• Minimum 2 years of nursing experience.
• BSN preferred; Medical Surgical/long-term care experience valued.
• Basic Life Support (BLS) certification required.

Responsibilities

• Assess, diagnose, plan, implement and evaluate patient care using the nursing process.
• Recognize changes in patient status and modify the plan or escalate appropriately.
• Advocate for patients and uphold autonomy per ANA code of ethics.
• Mentor colleagues and participate in governance of practice.
• Precept student nurses and other healthcare team members.

Overview

Registered Nurse (RN) - Long Term Care System Float

3x12 hour shifts - Straight Nights or D/N Rotation

This position is part of the System Staffing Office. Our Long-Term Care nurses are experienced inpatient nurses with Medical Surgical experience and who have agreed to be floated to Hanover Terrace,

within the sub-acute care unit.

Hanover Terrace (100 bed facility {50 are Long Term Care and 50 are subacute post hosptialized patients})

System Staffing Office RN's will care for subacute patients, not the LTC beds; Ratio is 8/9 patients p/each RN

Point Click Care is the documentation platform - preferred but not required

Responsibilities

• Utilizes the nursing process to assess, diagnose, identify outcomes, plan, implement and evaluate an individualized plan of care.
• Utilizes critical thinking and the nursing process to anticipate and recognize changes in patient status, taking action to modify the plan of care or to elevate to the care team as necessary.
• Practices in accordance with the ANA Code of Ethics to advocate for patients, uphold their autonomy in decision‑making, ensure informed consent and assist patients in families in expressing self‑determination.

Qualifications

• Graduate from an accredited Nursing Program required.
