Job summary

Momentum at South Bay for Nursing and Rehab in East Islip, NY is seeking a Per Diem RN Supervisor to join our night/day team. The RN will oversee the unit, ensure quality care, lead staff, and coordinate with physicians and families.

The role requires an active RN license, reliable transportation, and strong leadership and communication skills; schedule includes rotating shifts with possible full-time coverage as needed.

Responsibilities

• Assess, plan, implement and evaluate nursing care.
• Guide staff and ensure task completion.
• Monitor and report changes in residents' symptoms and behavior.
• Communicate with interdisciplinary team and physicians.
• Report to the Director of Nursing.

Momentum at South Bay has an opportunity for a Per Diem Registered Nurse Supervisor to join our team.

CPT approved site.

PRIMARY FUNCTIONS (Including but not limited to):

Our RN's are responsible for the overall operation and optimal quality of care for the residents on the assigned unit.

Follows all health, sanitary and infection control policies and maintains established standards of practice set forth by the facility's administration and Nursing Policies and Procedures.

DUTIES AND RESPONSIBILITIES:

• Contacts the resident's physician and family to notify them of a change in the resident's condition or accident/incident; also notifies the supervisor covering.
• Assumes all responsibilities of the RN Supervisor in his/her absence or as directed by the DNS/ADNS.
• Assumes independent responsibility for completing the required routine documentation and auditing as required.
• Must have strong communication skills and the ability to lead and motivate the team.
• Comply with all state requirements and facility policies, procedures and standards of conduct.
• Have the ability to assess, plan, implement and evaluate nursing care according to the individualized needs.
• Guides the staff and ensures tasks are being completed.
• Monitors and reports changes in residents’ symptoms and behavior.
• Communicates and collaborates with the interdisciplinary team and or physicians.
• Reports to the Director of Nursing

QUALIFICATIONS:

• Current RN license
• Reliable transportation and demonstrate excellent work ethic.
• Must have excellent interpersonal, communication and organizational skills

Job Type:

Full-time & Per diem

Salary:

$55-60 per hour

Schedule:

Open Availability

M-F, 3pm-11pm or 11-7pm, Rotating weekends (Full-time: 3-11)
