Job summary

Momentum at South Bay for Nursing and Rehab in East Islip, NY is seeking a Per Diem RN Supervisor to join our night/day team. The RN will oversee the unit, ensure quality care, lead staff, and coordinate with physicians and families.

Responsibilities

• Notify residents' physician and family of condition changes or incidents.
• Assume RN Supervisor responsibilities in absence or as directed.
• Complete routine documentation and auditing.
• Lead and motivate the nursing staff.
• Ensure compliance with state requirements and facility policies.
• Assess, plan, implement and evaluate nursing care.
• Guide staff and ensure task completion.
• Monitor and report changes in residents' symptoms and behavior.
• Communicate with interdisciplinary team and physicians.
• Report to the Director of Nursing.
