Job summary

Person Memorial Hospital in Roxboro, NC, part of Lifepoint Health, is seeking a Registered Nurse (RN) for a full-time night shift in the skilled nursing unit. You will provide direct patient care using the nursing process, administer medications, and collaborate with a multidisciplinary team to support a high standard of care for residents, including seniors with complex conditions.

The role emphasizes education for patients and families, meticulous documentation, and adherence to infection

Qualifications

• Current North Carolina RN license or compact license.
• Associate Degree in Nursing from an accredited program.
• Basic Life Support (BLS) certification from the American Heart Association is required on hire.
• One year of experience is preferred; new grads encouraged to apply.

Responsibilities

• Provide direct and indirect patient care using the nursing process (assessment, planning, implementation, evaluation).
• Plan for 24-hour care by assessing and documenting health status and treatment needs.
• Administer medications and treatments per policy; monitor patient responses.
• Communicate changes in patient condition to physicians and health care team; ensure orders are followed.

More about our team

This unit has 60 beds, with an average occupancy of 50 to 53 residents. We care for patients with diagnoses such as Dementia, Alzheimer's, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), and those in need of short-term rehabilitation. In addition, there are 26 private rooms. Our primary focus is on the senior population and conveniently located within Person Memorial Hospital (PMH). Nurses will collaborate closely with the laboratory, rehabilitation, hospice, and psychiatric teams.

Summary

Provides direct and indirect patient care utilizing the nursing process of assessment, planning, implementation and evaluation. Meets the physical, psychosocial, religious/cultural and needs of the patient and family. Communicates effectively with all members of the health care team as appropriate about changes in the patient's condition or response to treatment.

How you'll contribute

• Provides patient/family education utilizing the teach back method, evaluates and recommends additional education as indicated by evaluation of patient/family learning and documents.
• Plan for 24hr care by assessing, monitoring, and documenting level of functioning, health status. mental status, medical history, medication compliance and treatment needs.
• Treats patients and their families with respect and dignity. Identifies and address psychosocial, cultural, ethnic and religious/spiritual needs of the patients and their families.
• Communicates clearly and professionally with all members of the health care team.
• Notifies physicians in a timely manner of changes in the patient's condition and response to treatment.
• Delivers patient care activities according to standards of Care and Practice.
• Administration of medications and treatments, i.e., initiation and maintenance of IV meds, blood and blood products,
• Checks emergency equipment/drugs for proper operation and availability every shift.
• Maintains a safe patient and working environment by adhering to infection control and safety policies and procedures.
• Receives physician orders, documents, follows up and evaluates patients' outcomes. Assures that orders are carried out properly per policy.
• Assist physician with bedside procedures.
• Perform other related duties incidental to work described herein.

Non-Essential Functions

• Assist with AM care and bed bath.
• Attends and participates in staff meetings, in-services, projects and committees as assigned.
• Requesting and putting supplies away
• Key in work orders
• Order daily dietary supplies
• Receptionist duties
• Work scheduled shifts and overtime as necessary

What we're looking for

• Current RN license in the state of North Carolina (or compact license)
• Associate Degree in Nursing (ASN) from an accredited nursing program
• Basic Life Support (BLS) certification from the American Heart Association is required upon hire.
• One year of experience is preferred (new grads encouraged to apply)
• Active North Carolina RN license (single state) or compact required. If relocating, licensure must be updated to the new Primary State of Residence (PSOR) before employment begins
