Job summary

Person Memorial Hospital in Roxboro, NC is seeking a Registered Nurse (RN) for a Skilled Nursing unit. This full-time role offers varied 12-hour shifts and a sign-on bonus up to $20,000 committed for two years.

You will deliver direct and indirect patient care, educate patients and families, and work with a multidisciplinary team to support residents with dementia, CHF, COPD and rehab needs. North Carolina nursing license required; ASN degree; BLS required. Benefits are included.

Qualifications

• Current NC RN license (or compact)
• ASN from an accredited nursing program
• BLS certification from the American Heart Association is required upon hire
• One year of experience is preferred (new grads encouraged to apply)
• Active NC license (single state) or compact; if relocating, license must be updated before employment

Responsibilities

• Provides patient/family education utilizing teach-back method and documents.
• Assess, monitor, and document level of functioning and health status.
• Communicate clearly with health care team about changes in patient condition.
• Notify physicians promptly of changes in condition and treatment response.
• Administer medications and treatments per policy and standards of care.
• Assist with bedside procedures as needed.
• Maintain infection control and safety policies to ensure a safe environment.

More about our team

This unit has 60 beds, with an average occupancy of 50 to 53 residents. We care for patients with diagnoses such as Dementia, Alzheimer's, Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), and those in need of short-term rehabilitation. In addition, there are 26 private rooms. Our primary focus is on the senior population and conveniently located within Person Memorial Hospital (PMH). Nurses will collaborate closely with the laboratory, rehabilitation, hospice, and psychiatric teams.

Summary

Provides direct and indirect patient care utilizing the nursing process of assessment, planning, implementation and evaluation. Meets the physical, psychosocial, religious/cultural and needs of the patient and family. Communicates effectively with all members of the health care team as appropriate about changes in the patient's condition or response to treatment.

Reports to: Unit Director

How you'll contribute

• Provides patient/family education utilizing the teach back method, evaluates and recommends additional education as indicated by evaluation of patient/family learning and documents.
• Plan for 24hr care by assessing, monitoring, and documenting level of functioning, health status. mental status, medical history, medication compliance and treatment needs.
• Treats patients and their families with respect and dignity. Identifies and address psychosocial, cultural, ethnic and religious/spiritual needs of the patients and their families.
• Communicates clearly and professionally with all members of the health care team.
• Notifies physicians in a timely manner of changes in the patient's condition and response to treatment.
• Delivers patient care activities according to standards of Care and Practice.
• Administration of medications and treatments, i.e., initiation and maintenance of IV meds, blood and blood products,
• Checks emergency equipment/drugs for proper operation and availability every shift.
• Maintains a safe patient and working environment by adhering to infection control and safety policies and procedures.
• Receives physician orders, documents, follows up and evaluates patients' outcomes. Assures that orders are carried out properly per policy.
• Assist physician with bedside procedures.
• Perform other related duties incidental to work described herein.

Non-Essential Functions

• Assist with AM care and bed bath.
• Attends and participates in staff meetings, in-services, projects and committees as assigned.

• Receptionist duties
• Work scheduled shifts and overtime as necessary

What we're looking for
