Liberty Rehabilitation Hospital, part of Lifepoint Health, is hiring a Registered Nurse (RN) for full-time day shifts in Liberty Township, OH. You will provide direct patient care using the nursing process, perform assessments, and support patient and family education.

Qualifications

• Associate's degree in nursing required.
• Current RN license as required by state regulations.
• CPR/BCLS certification; ACLS preferred.

Responsibilities

• Provides direct patient care using the nursing process.
• Administers medications and performs specified clinical tasks.
• Documents care, monitors patient condition, and communicates with the care team.
• Assists with activities of daily living and patient education.
• Collaborates across disciplines for transfers, discharge, and psychosocial needs.
• Advocates for physical and emotional well-being of patients.

How You'll Contribute

Provides direct patient care to patients using the nursing process in accordance with applicable scope and standards of practice.

• Performs plan of care intervention, including medication administration, specimen collection, clinical treatments, as well as other medical care treatment.
• Documents patient care given.
• Monitors, records, and communicate patient condition as appropriate to care team, physician, patient, and family.
• Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
• Collaborates as needed across disciplines to coordinate patient care, including patient transfer, discharge, referral and spiritual/psychosocial support needs.
• Evaluates learning needs of patient and/or family and provides patient/family education appropriate to age, culture, condition, and circumstances.
• Works as an advocate for the physical and emotional well‑being of the patient.

What We're Looking For

Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action.

• Associate's degree in nursing required.
• Current Registered Nurse license as required by state regulations in which the facility operates.
• CPR/BCLS certification. ACLS preferred.
• Excellent oral and written communication and interpersonal skills.
• CRRN preferred
