Job summary

Caremed, Inc. in Virginia seeks compassionate Registered Nurses (RN) for PRN and Full-Time roles delivering one-on-one care in patients' homes across Amelia, Buckingham, Cumberland, Nottoway, Powhatan, and Prince Edward counties.

You will perform home visits, assessments, and patient education, collaborate with care teams, document visits, and mentor home care aides. Active Virginia RN license, ADN/BSN, CPR/BLS, and reliable transport required; 1 year home health experience preferred.

Qualifications

• Active RN license in Virginia in good standing.
• Graduate of an accredited nursing program (ADN or BSN).
• Current CPR/BLS certification.
• 1 year of RN home health/home care experience preferred.
• Valid driver’s license and reliable transportation.
• Strong clinical judgment and excellent communication.

Responsibilities

• Home Visits: Conduct regular, personalized visits to evaluate and support patients' needs.
• Comprehensive Assessments: Thoroughly assess physical, psycho-social, and environmental factors in the home.
• Patient & Family Education: Empower families with clear guidance and resources to enhance quality of life.
• Collaborative Care: Partner with multidisciplinary healthcare teams to ensure seamless care delivery.
• Documentation & Supervision: Maintain timely and accurate visit documentation, and provide guidance to home care aides.
