Rose International is seeking a Registered Nurse (RN) for Primary Care Outpatient to join our onsite Cedar Rapids team. The role emphasizes high-touch, team-based primary care, care coordination, and proactive patient outreach in a non-acute outpatient setting.

The RN will support care team decisions, participate in weekly care team meetings, provide patient education, coordinate services, and help ensure high-quality, prevention-focused care.

Registered Nurse (RN) – Primary Care Outpatient Overview
  • The Registered Nurse (RN) – Primary Care Outpatient plays an important role in delivering high-touch, team-based primary care and coordinating patient care throughout the healthcare delivery system
  • The Registered Nurse (RN) – Primary Care Outpatient will build strong relationships with patients and their caregivers while managing care plans and providing proactive telephone and in-person outreach based on patient needs. This position requires flexibility, attention to detail, strong problem-solving skills, and a commitment to delivering an exceptional patient experience
  • The RN will support care team decision-making, participate in weekly care team meetings, provide patient education, coordinate clinical and auxiliary services, and help ensure patients receive high-quality, prevention-focused care
  • The role also includes telephone support and triage, symptom management, care coordination, medication and testing authorizations, and collaboration with Medical Assistants and other members of the care team
  • The RN may periodically perform home visits and will be expected to demonstrate positive energy, accountability, clinical excellence, and a strong commitment to patient-centered care
