Job summary

Pillar of Cedar Valley Nursing and Rehabilitation is seeking an RN or LPN to provide compassionate resident care and to lead and mentor the CNA team. You will implement individual care plans, administer medications, and coordinate admissions, transfers, and discharges.

Responsibilities

• Be responsible for the care and well being of the residents in your charge and mentor the CNA’s on your team
• Assess residents’ needs and assist in developing and following individual care plans
• Assure quality care through use of nursing processes and guidelines
• Identify and communicate changes in residents’ status to physicians, staff, and family
• Administer medications, treatments, and other direct care as prescribed
• Coordinate admissions, transfers, and discharges of residents
• Communicate appropriate information to all staff for smooth shift transitions
• Investigate and document incidents and accidents as required
• Report any suspected abuse to appropriate authority
• Document appropriately as needed in PCC
• Push/pull 30 pounds of force to move medication carts and transport residents in wheelchairs
