POSITION SUMMARY

The On-Call RN provides emergency support and skilled visits to patients on an on-call basis during non-standard work hours.

ESSENTIAL FUNCTIONS

• Make emergency on-call visits demonstrating the skills, knowledge, and judgment necessary to implement the nursing care plan; document appropriately to meet Medicare conditions of participation; provide quality care through symptom management and emotional support.
• Participate in weekly team conferences by contributing necessary information and documentation.
• Responsible for Hospice patients during non-office hours, providing a feeling of security through phone communication and skilled visits as indicated.
• Assume responsibility for attendance, dependability, and personal appearance with minimal supervision.
• Be capable of handling interpersonal relationships, able to communicate effectively, and demonstrate professional development.
• Be able to use, store, and care for equipment, use safety in work habits, and respond properly in crises.
• May be required to evaluate appropriateness for hospice care and admit patients.

QUALIFICATIONS

• Registered Nurse in the state of Texas
• Hold a valid driver’s license and have a dependable means of transportation
• Minimum 2 years of RN experience required
• 6+ months in an On-Call role required
• Crisis management experience strongly preferred
• 1 year Hospice or Home Health experience strongly preferred
• Proficient computer skills are required; Microsoft Office suite experience preferred.
