Job summary

Trinity Health’s St. Mary Medical Center in Bucks County, PA seeks a Registered Nurse for full-time night shifts. The RN is responsible for assessing, planning, implementing, and evaluating patient care, with delegation to the healthcare team as needed.

Responsibilities include documenting assessments, coordinating care through information systems, and ensuring safety and infection control standards. Requires PA RN license and BLS; ASN/ADN required, BSN preferred.

• AHA BLS certification.

Responsibilities

• Assess learning needs, readiness to learn, and identify barriers to learning; tailor education to patient’s age, culture, and values.
• Document admission assessments, reassessments, plan, interventions, and medication use with outcomes.
• Use information systems to communicate changes in patient condition to the care team.
• Coordinate transfer of patient responsibilities during bedside handoffs and inter-professional rounds.
• Recognize patient unique needs and refer for abuse as needed; incorporate pain management in care plans.
• Maintain infection control standards and a safe, clean environment; perform other duties as assigned.

The Registered Nurse (RN) is responsible for assessing, planning, implementing and evaluating the delivery of patient care. The professional nurse assumes the responsibility and accountability for the delegation of patient care to other members of the healthcare team.

Job Summary:

• Assesses learning needs, readiness to learn and identify actual or potential barriers to learning and thereby incorporating the patient’s education into daily activities based on age, cultural and spiritual values.
• Documents admission assessment / reassessment, plan, interventions and medication use and monitors its effects and outcomes according to policy.
• Utilizes the information management system to enhance communication and documents change in-patients condition to all appropriate individuals. Prioritizes work assignment based on changing situations.
• Communicates transfer of patient responsibility and pertinent information at the bedside and during Interprofessional Rounds
• Recognizes and integrates patient special needs and individualizes care and knowledgeable in abuse recognition and referral
• Includes pain management and supportive care issues in the plan of care.
• Adheres to infection control standards specific to patient care and by promotes and maintains a safe, clean and orderly environment.
• Performs other duties as assigned.
