Registered Nurse (RN), Mobile MDS Coordinator
CommuniCare Health Services is currently seeking a Registered Nurse (RN) to fill the key position of Mobile MDS Coordinator across its skilled nursing facilities in the Midwest Division.
The ideal candidate must possess a valid RN license in Ohio and have experience in Long Term Care and MDS. Multi-facility or Senior MDS experience is preferred. The Mobile MDS Coordinator should be detail-oriented, a team player, and able to communicate effectively with residents, families, and staff at all levels. The role involves travel throughout Ohio and occasionally to Indiana, with facilities located in Cincinnati, Dayton, Columbus, Akron, Youngstown, Cleveland, and Toledo areas.
