Job summary

Family Pillars Homecare, LLC is seeking dedicated RNs and LPNs to provide in-home skilled nursing care in Florida on a per diem basis. You will perform skilled visits, coordinate care plans, and collaborate with physicians to support patients at home.

Required are a Florida RN or LPN license (Compact accepted), current BLS, AHCA background check, TB screening, and experience with Medicare-certified home health. Proficiency with Axxess EMR and knowledge of PDGM &OASIS are a plus.

Qualifications

Florida RN or LPN license (Compact accepted)
Current CPR certification (BLS)
Level 2 background screening (AHCA-compliant)
TB screening and physical (within past 6 months)
Experience in Medicare-certified home health strongly preferred
Familiarity with EMRs (Axxess) and knowledge of PDGM & OASIS for RNs
Excellent clinical judgment, communication skills, and time management
Reliable transportation and commitment to punctual, professional care

Responsibilities

Perform skilled nursing visits: wound care, IV administration, medication management, patient assessments
Carry out shift-based care for patients needing extended clinical support
Monitor and document patient condition and progress
Collaborate with physicians and interdisciplinary teams to update care plans
Maintain accurate records in compliance with HIPAA and CMS guidelines (OASIS for RN)
Uphold infection control standards and agency protocols
