Responsibilities

• Deliver 1-on-1 skilled nursing care and monitoring during overnight hours based on individualized client care plans.
• Administer specialized nursing interventions within scope (PICC line management, port accessing, peripheral IV insertion).
• Support clients and families with health education, medication administration, and compassionate guidance.
• Complete timely clinical documentation and report status changes to interdisciplinary care teams.
• Participate in periodic case management conferences and team check-ins.
