Job Summary

This is a per-visit position, ideal for an RN who wants control over their schedule while still doing meaningful, hands‑on patient care. You will create and manage individualized plans of care for home health and hospice patients, working independently in patients' homes while staying connected to a supportive clinical team. You are detail oriented with a big heart, take pride in personalized care planning, and communicate effectively in your patient's best interest.
