Job summary

Transitions Hospice is seeking an experienced RN for the Extended Care Team (ECT) as a per diem Hospice RN visiting patients in Columbus, OH. The role involves flexible weekend rotations and daytime visits, providing expert nursing care and rapid assessment in a hospice setting.

You will join an interdisciplinary team, document thoroughly, and coordinate with physicians to ensure patient comfort and timely care. Valid OH RN license and travel willingness required.

Qualifications

• Active RN license in OH
• Graduate of an accredited school of Nursing
• Hospice/home health experience preferred
• Flexible and confident across varied patient assignments
• Strong assessment and clinical judgment under pressure
• Excellent documentation habits and communication skills
• Valid driver's license and willingness to travel locally

Responsibilities

• Deliver professional nursing care across the full nursing process within scope of practice
• Conduct patient and family assessments and contribute to care plans
• Complete all visit types as needed to meet patient/family needs
• Administer medications and treatments per physician orders
• Identify nursing diagnoses from assessments and respond accordingly
• Record accurate, timely clinical notes including pain and symptom outcomes
• Communicate changes in patient condition to physicians and obtain orders as required
• Coordinate with community health partners to support seamless care transitions
• Ensure equipment and services are arranged and available for patients
• Maintain and grow clinical skills through continuing education and in-service participation

What You'll Do

As a Transitions Hospice ECT Nurse, you'll work as a vital support member of the interdisciplinary care team — stepping in across cases to assess, treat, and communicate patient needs with speed and clinical precision. Your responsibilities include:
