VitalCaring Group is seeking a Hospice Registered Nurse to lead patient-centered hospice care in field settings. You will manage care plans, assess patients and families, and coordinate with physicians, social workers, and aides to ensure comfort and dignity at end of life.

This role emphasizes autonomy with team support, prioritizing compassionate, personalized care, and strong communication in sensitive situations across diverse home and facility settings.

Qualificações

• Active RN license (or compact eligibility) and nursing degree.
• Minimum 1 year of clinical nursing experience.
• Strong clinical judgment and compassionate care approach.
• Ability to manage a caseload independently.
• Excellent communication skills in sensitive situations.
• Basic EMR proficiency and reliable transportation.

Role Overview

As a Hospice RN at VitalCaring, you are more than a nurse - you are the clinical leader, advocate, and steady guide for your patients and their families.

You’ll lead the plan of care, manage symptoms, and ensure every patient receives personalized, comfort-focused care aligned with their goals and wishes. Through each visit, you bring not only clinical expertise, but also reassurance, clarity, and compassion.

What You’ll Do

Lead Patient-Centered Hospice Care

• Conduct comprehensive patient and family assessments
• Develop and manage individualized, comfort-focused care plans
• Deliver skilled nursing care with a focus on pain and symptom management

Serve as the Care Coordinator

• Act as case manager, coordinating care across the interdisciplinary team
• Collaborate with physicians, social workers, chaplains, and aides
• Monitor patient condition and adjust care plans to maintain comfort

Support Patients & Families

• Provide education, emotional support, and guidance throughout the journey
• Help families navigate complex decisions with empathy and clarity
• Build meaningful, trusting relationships in deeply personal moments

Drive Quality & Compliance

• Maintain accurate, timely documentation
• Participate in IDT meetings and care planning
• Support quality and compliance standards

What You Bring

• Active RN license (or compact eligibility)
• Graduate of an accredited nursing program
• Minimum 1 year of clinical nursing experience
• Strong clinical judgment and compassionate care approach
• Ability to manage a caseload independently
• Excellent communication skills in sensitive situations
• Basic EMR proficiency
• Valid driver’s license and reliable transportation
