VitalCaring Group is seeking a Hospice Registered Nurse (RN) to deliver compassionate, one-on-one end-of-life care in patients’ homes and facility settings. You will lead the plan of care, manage symptoms, and advocate for patients while coordinating with physicians, social workers, chaplains, and aides.

You will provide education and emotional support to families, ensuring comfort, dignity, and alignment with patient goals.

Qualifikationen

• Active RN license (or compact eligibility)
• Graduate of an accredited nursing program
• Minimum 1 year of clinical nursing experience
• Strong clinical judgment and compassionate care approach
• Ability to manage a caseload independently
• Excellent communication skills in sensitive situations
• Basic EMR proficiency
• Valid driver’s license and reliable transportation

Role Overview

As a Hospice RN at VitalCaring, you are more than a nurse - you are the clinical leader, advocate, and steady guide for your patients and their families.

What You’ll Do

Lead Patient-Centered Hospice Care

• Conduct comprehensive patient and family assessments
• Develop and manage individualized, comfort-focused care plans
• Deliver skilled nursing care with a focus on pain and symptom management

Serve as the Care Coordinator

• Act as case manager, coordinating care across the interdisciplinary team
• Collaborate with physicians, social workers, chaplains, and aides
• Monitor patient condition and adjust care plans to maintain comfort

Support Patients & Families

• Provide education, emotional support, and guidance throughout the journey
• Help families navigate complex decisions with empathy and clarity
• Build meaningful, trusting relationships in deeply personal moments

Drive Quality & Compliance

• Maintain accurate, timely documentation
• Participate in IDT meetings and care planning
• Support quality and compliance standards

What You Bring

• Active RN license (or compact eligibility)
• Graduate of an accredited nursing program
