Trellis Supportive Care in Salisbury, NC is seeking a Hospice RN Case Manager – Homecare to deliver compassionate hospice care in patients' homes and care facilities. You will develop individualized plans of care and coordinate with an interdisciplinary team to ensure comfort and quality of life.

Location: Serving patients and families in Rowan County and surrounding counties
Office Location: Trellis Supportive Care – Salisbury, NC

Schedule

• Full-Time (40 hours per week)
• Five 8-hour weekday shifts
• Occasional back-up on-call rotation
