Job summary

Trellis Supportive Care in Winston-Salem, NC is seeking a Registered Nurse (RN) Hospice Case Manager for homecare. The role provides compassionate hospice care to patients in their homes and facilities, developing and implementing individualized plans of care.

You will coordinate care with an interdisciplinary team, perform reassessments, and ensure comfort and quality of life while upholding our values of Patient & Family Commitment, Professionalism, and Clear Communication.

Location: Serving patients and families in Forsyth County and surrounding counties
Office Location: Trellis Supportive Care – Winston Salem, NC

Schedule

• Full-Time (40 hours per week)
• Five 8-hour weekday shifts
• Occasional back-up on-call rotation
