Job summary

Transitions Hospice is seeking a Night Nurse (RN) to deliver professional nursing care during overnight hours (8pm to 8am) across Will, Kendall, DuPage, Kane, DeKalb, Lee, Ogle, Cook and surrounding counties.

You will perform assessments, contribute to individualized care plans, administer medications per orders, record clinical notes, communicate changes to physicians, and coordinate with community partners to ensure continuity of care and patient comfort.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited School of Nursing.
• Clinical experience in hospice, home health, or related setting preferred.
• Comfort and composure working independently during overnight hours.
• Strong assessment skills and sound clinical judgement.

Responsibilities

• Delivering professional nursing care across the full nursing process within your scope of practice.
• Conducting thorough patient and family assessments and contributing to individualized care plans.
• Completing all visit types as needed to meet overnight patient and family needs.
• Administering medications and treatments per physician orders.
• Identifying nursing diagnoses from health assessment data and responding with sound clinical judgment.
• Recording accurate, timely clinical notes including pain and symptom management outcomes.
• Communicating changes in patient condition to physicians and obtaining orders as required.
• Coordinating with community health partners to support continuity of care.
• Ensuring equipment and necessary services are in place for patients.
• Maintaining and growing your clinical skills through continuing education and in-service participation.
