Phoenix Hospice in Springfield, MO is seeking a dedicated RN Case Manager to join our compassionate team. You'll deliver skilled hospice nursing care, educate families on care plans, and guide goals-of-care discussions with empathy.

Qualifications

• Active Missouri or Compact RN license required.
• Hospice or Long-Term Care nursing experience preferred.
• Strong assessment, communication, and care coordination skills.

Responsibilities

• Provide compassionate hospice nursing care to patients and families.
• Educate patients and caregivers on the hospice process and plan of care.
• Guide families through goals-of-care discussions and healthcare decisions.
• Conduct initial and ongoing comprehensive assessments.
• Perform hospice evaluations and informational visits.
• Collaborate with physicians, Hospice Medical Directors, and interdisciplinary team members.
• Coordinate care to ensure patient and family needs are fully met.
• Communicate changes in patient condition and update plans of care as needed.
