Job summary

Compassus in Albuquerque seeks a Hospice Admission Registered Nurse to assess hospice eligibility, explain services to patients and families, and initiate the plan of care with the Inter-Disciplinary Team.

Qualifications

• RN license in the state you work.
• Two years of RN experience; hospice experience preferred.
• Valid driver's license.
• CPR and BCLS certification.

Responsibilities

• Enable patients to spend quality time with loved ones and activities they enjoy.
• Assess patients for hospice eligibility and explain services to patients, families/caregivers.
• Ensure all patient’s initial needs are addressed.
• Collaborate with the Inter-Disciplinary Team on initial plan of care and patient status.

• Enable patients to spend quality time with their loved ones and doing the activities they enjoy
• Assess patients for hospice eligibility and explain hospice care services to patients, families/caregivers
• Ensure all patient’s initial needs are addressed
• Collaborate with the Inter-Disciplinary Team members regarding initial plan of care, patient’s condition, and other essential information

Requirements

• RN license in the state you work
• Two years of experience as a registered nurse, hospice experience preferred
