Job summary

Compassus is seeking a Hospice Admission Registered Nurse to join a team dedicated to compassionate home-based care. You will assess patients for hospice eligibility, explain services, and address initial needs while coordinating with an Inter-Disciplinary Team to craft an effective plan of care.

Qualifications

• Minimum two years of registered nurse experience; hospice experience preferred.
• Current RN license in the state you work.
• Current CPR/BCLS certification and driver's license required.

Responsibilities

• Assess patients for hospice eligibility and explain services to patients and families.
• Ensure initial patient needs are addressed and documented.
• Collaborate with the Inter-Disciplinary Team on plan of care and patient condition.

How you’ll make an impact

• Enable patients to spend quality time with their loved ones and doing the activities they enjoy
• Assess patients for hospice eligibility and explain hospice care services to patients, families/caregivers
• Ensure all patient’s initial needs are addressed
• Collaborate with the Inter-Disciplinary Team members regarding initial plan of care, patient’s condition, and other essential information

Requirements

• RN license in the state you work
• Two years of experience as a registered nurse, hospice experience preferred
• Current driver’s license
• Current CPR and BCLS certification
