Job summary

Compassus in Louisiana is seeking a Hospice Admission Registered Nurse (RN) to assess patients for hospice eligibility and coordinate initial care plans. You will collaborate with the Inter-Disciplinary Team to ensure timely initiation of services and support families during transition.

The role requires an active RN license in the state, at least two years of RN experience (hospice preferred), a valid driver’s license, and CPR/BCLS certification.

• Two years of experience as a registered nurse, hospice experience preferred.
• Current driver’s license.
• Current CPR and BCLS certification.

Responsibilities

• Enable patients to spend quality time with their loved ones and doing the activities they enjoy.
• Assess patients for hospice eligibility and explain hospice care services to patients, families/caregivers.
• Ensure all patients’ initial needs are addressed.
• Collaborate with the Inter-Disciplinary Team members regarding initial plan of care, patient’s condition, and other essential information.
