Job description

Role: Registered Nurse (RN) Hospice, providing specialized end-of-life care in patients' homes or facilities.

• Responsibilities: Assess patient needs, develop and implement personalized care plans, collaborate with healthcare teams, promote culturally sensitive care, evaluate patient condition changes, and communicate updates. Support patients and families emotionally, spiritually, and physically, ensuring quality and evidence-based care while maintaining safety standards.
• Qualifications: Nursing diploma or associate's degree, RN license, valid driver's license, less than 2 years experience preferred, with a bachelor's degree and 2-5 years of relevant experience preferred. Prior hospice or admissions experience is advantageous.
• Skills: Strong assessment, communication, cultural competency, teamwork, and critical thinking skills. Ability to work independently and handle sensitive situations with empathy and professionalism.
• Additional Duties: Support community travel, participate in a set schedule with weekend options, and contribute to holistic patient care through interdisciplinary teamwork.
