Job summary

Transitions Hospice is seeking an RN Case Manager in Peoria, IL to coordinate care and lead the interdisciplinary team in delivering compassionate hospice services in patients' homes. You will conduct comprehensive assessments, create individualized care plans, administer treatments per orders, and ensure clear communication with families and physicians.

Active RN license required in Illinois, with 1+ year hospice/home health experience preferred.

Qualifications

• Active RN license in the state of employment.
• Graduate of an accredited nursing program.
• 1+ year hospice, home health, or related experience preferred.
• Compassion for patients and families navigating end-of-life care.

Responsibilities

• Coordinate care as the clinical anchor for patients and families.
• Guide the interdisciplinary team and participate in on-call rotation.
• Conduct comprehensive patient assessments and build personalized care plans.
• Administer medications and treatments per physician orders.
• Maintain accurate, timely clinical documentation and communication between families, physicians, and partners.

Schedule: 8 AM–8 PM, alternating 4-day and 3-day weeks including weekends in rotation

Salary Range: $75,000 - $85,000 a year

What You'll Do

As an RN Case Manager, you'll be the clinical anchor for your patients and their families — coordinating care, guiding the interdisciplinary team, and showing up when it matters most. Your day-to-day includes:

• Conducting comprehensive, ongoing assessments to understand each patient's evolving needs
• Building and adjusting personalized care plans in collaboration with physicians and care teams
• Administering medications and treatments per physician orders
• Keeping clinical documentation accurate, timely, and meaningful
• Serving as the communication bridge between families, physicians, and community health partners
• Educating patients and caregivers so they feel confident and supported
• Participating in on-call rotation per team policy
