Job summary

Sequoia Hospice is seeking an experienced Registered Nurse (RN) to join our Hospice Care team. This part-time position focuses on weekend coverage in home-based settings across Alameda, Santa Clara, and Peninsula regions.

You will plan, organize, and direct hospice care, act as case manager, and educate patients and families with empathy and professionalism. CA RN license, CPR, and strong clinical skills are required.

Qualifications

  • Active California RN license required.
  • Minimum 2 years nursing experience (hospice or home health preferred).
  • CPR certification required.

Responsibilities

  • Assess patients and families to plan hospice care.
  • Develop and revise individualized care plans.
  • Provide nursing care using the nursing process.
  • Administer medications and treatments as prescribed.
  • Educate families on hospice care and symptoms.
  • Coordinate care for assigned caseload.

What You’ll Do

As a Hospice RN, you will plan, organize, and direct hospice care while educating patients and families. You’ll serve as a case manager and ensure quality, compassionate care in the home setting.

Key Responsibilities
  • Complete initial and ongoing patient and family assessments
  • Develop and revise individualized hospice care plans
  • Provide professional nursing care using the nursing process
  • Administer medications and treatments as prescribed
  • Educate and counsel patients and families on hospice care
  • Coordinate care for your assigned caseload
What We’re Looking For
  • Active CA RN License
  • Minimum 2 years of nursing experience (hospice or home health experience preferred)
  • Strong clinical, communication, and problem-solving skills
  • Current CPR certification
  • Valid driver’s license, insured vehicle in good condition
  • Ability to work independently and manage priorities
