Job summary

The Sequoia Hospice team in Santa Clara County is seeking a compassionate Hospice Aide (HHA) to provide comfort and support to patients and families. You will work under the supervision of a registered nurse to deliver essential personal care and aid in daily activities within a caring home health setting.

This full-time role requires a California HHA certification, CPR, and a valid driver’s license. You’ll contribute to a patient-centered mission and join a dedicated interdisciplinary care team

Qualifications

• Current California Home Health Aide Certification (required).
• CPR certification and a valid driver’s license with reliable, insured transportation (required).
• Ability to read and follow care plans and document services accurately (required).
• Understanding of hospice philosophy and comfort working with terminally ill patients (required).
• Physical ability to lift, transfer, and assist patients safely (required).

Responsibilities

• Delivering personal care such as bathing, grooming, oral hygiene, and dressing assistance.
• Supporting mobility and exercise as directed by the care team.
• Monitoring and reporting patient observations to ensure safety and comfort.
• Assisting with feeding, light laundry, and other daily needs.
• Taking vital signs when ordered and maintaining accurate documentation.
• Participating in quality improvement activities to help us continually raise the bar in care.

What You’ll Do
