Job summary

Deercreekseniorliving is seeking an experienced, compassionate Registered Nurse (RN) to provide home-care services. This role offers a pay range of $62 – $70 an hour based on experience. The RN will develop care plans alongside patients and families, provide essential nursing services, and ensure thorough documentation and communication with healthcare teams.

Successful candidates will have graduated from a nursing program, possess a valid nursing license, and ideally have experience in public health or home-care nursing. The position is available on a full-time, part-time, or per diem basis.

Qualifications

  • Minimum 2 years of experience in nursing, 1 year in public health or home-care preferred.
  • Current CPR certification.
  • Licensed driver with an insured automobile.

Job Summary

Sequoia Hospice – East Bay is seeking an experienced, compassionate Registered Nurse (RN) to provide home‑care services. Pay Range: $62 – $70 an hour. Job Type: Full Time, Part Time, Per Diem.

Duties & Responsibilities
  • Develop and initiate the plan of care and assume responsibility for ongoing interdisciplinary assessment and development of individualized care plans in partnership with patients, representatives and caregivers.
  • Initiate proper preventative and rehabilitative nursing procedures and provide services ordered by the physician as indicated in the plan of care.
  • Provide patient, caregiver and family counseling and education.
  • Prepare clinical notes and communicate with the physician and other health‑care practitioners regarding the home‑health plan of care.
  • Identify discharge‑planning needs as part of the care plan development and implement them prior to patient discharge.
  • Maintain up‑to‑date clinical documentation and coordinate with other professionals as needed.
Job Requirements
  • Graduated from an approved school of nursing and licensed in the state where practicing.
  • Minimum of two years of experience, at least one year in public health or home‑care nursing preferred.
  • Maintain a current CPR certification.
  • Licensed driver with an insured automobile in good working order.
  • Self‑directed, able to work with minimal supervision, and able to organize and prioritize workload independently.
  • Excellent observation, problem‑solving, verbal and written communication skills; nursing skills per competency checklist.
  • Responsible for supervising home‑health aides.
  • Physical capability to stand, walk, lift, transfer patients and heavy supplies, and to reach, stoop, kneel, crouch, and manipulate equipment.
