Join our growing team! We have a full-time Registered Nurse (RN) position available for Home Health Visits in Senior Living communities in Harrisburg and surrounding areas.

Responsibilities:

• Follow care plans tailored to patient needs.
• Make home visits within designated geographic areas.
• Administer medication, perform wound care, treatments, and procedures.
• Monitor client conditions and report changes.
• Follow up on and document doctors' orders.
• Perform client assessments as needed.
• Coordinate case management and document care using electronic medical records.

Qualifications:

• Current Pennsylvania RN license.
• Minimum of one year of recent clinical experience.
• Home care experience preferred but not required.
• Graduation from an accredited nursing program.
• Ability to work independently and manage time effectively.
• Strong interpersonal and computer skills; EMR experience preferred.
• Ability to travel to assigned cases.
