Registered Nurse Home Health PRN

Coverage 1: Edwardsville

Coverage 2: Belleville, East St. Louis

Key Responsibilities

• Ensure all activities align with Elara Caring’s vision and leadership directives.
• Assess patients’ physical, psychosocial, and environmental needs through documentation and evaluations.
• Manage caseload including assessment, planning, coordination, and evaluation of treatment plans.
• Gather data on patient needs and incorporate findings into the care plan.
• Perform initial and ongoing evaluations, and follow-up visits based on patient needs.
• Communicate significant findings and health changes to the team and healthcare providers.
• Revise care plans as needed and notify relevant team members.

Requirements

• Current, unrestricted state RN license
• At least 1 year of clinical nursing experience
• Willingness and ability to travel within the coverage area
• Dependable vehicle, valid driver’s license, auto insurance
• Willing to travel up to 50%
• Associates or Bachelor’s Degree in Nursing preferred
• Home Health or Hospice experience preferred
• Experience with HomeCare HomeBase preferred

The compensation ranges from $45.00 to $55.00 per hour, with potential additional earnings for extra work, bonuses, or performance-based variables.

You will report to the Clinical Manager or Supervisor.
